What is the Farmacy?
Appalachian Farmacy seeks to improve community health outcomes by making fresh fruits and vegetables more accessible to local community members.
To participate in Appalachian Farmacy, community members must meet both of the following eligibility requirements:
qualify for SNAP, Medicaid, or Medicare
have high blood pressure or obesity as determined by a health care provider
Once eligibility is determined, participants must attend an program orientation session. At orientation, participants will be given a welcome packet with all program information, and complete the program onboarding survey. The onboarding survey securely records participants health information through the ETSU College of Public Health’s survey system, Qualtrics. Your health information will be anonymized and will not be shared with anyone. As part of the onboarding process at orientation, blood pressure measurements will be recorded on site.
The Appalachian Farmacy program is currently full for 2025.

Determine Your Eligibility: Participants must qualify for SNAP or Medicaid/Medicare AND have high blood pressure or obesity.
Complete an onboarding survey provided by the ETSU College of Public Health.
Take what you learned and your fresh produce home to make healthy meals! Follow-ups will be done throughout the course of the program to ensure participants are staying on track.
The tokens can be spent at any market booth on fresh fruits and vegetables.
